Description: The source bank is a collection of commonly used tools that support your HDCGP evaluation plan. ResultsLab has gathered together resources that will help you align your project to Social Determinant of Health outcomes, capture meaningful data through validated indicators, and communicate your work to CDPHE and other stakeholders. The source bank is an evolving resource that is reviewed and/or updated quarterly as ResultsLab consultants identify new resources and as HDCGP grantees discover helpful tools for your work.
